Job Summary:

House Calls is an interdisciplinary primary healthcare team providing ongoing comprehensive geriatric home-based primary care since 2008. House Calls provides comprehensive primary medical care, as well as occupational therapy, physiotherapy and social work. House Calls also facilitates connections to community support services for homebound seniors with physical, cognitive and social frailties. House Calls and its partners also work closely with hospitals and other health care providers to foster client-centred care, especially during transitions between care providers. www.seniorshousecalls.ca

The Coordinator will be the initial point of contact for House Calls; this position will take responsibility for screening calls and determining needed services/supports to which clients (and their families) can be referred. The Coordinator will be responsible for administrative and service responsibilities such as staff scheduling, managing fax referrals and filing. The Coordinator utilizes their knowledge of medical terminology and medical ethics, and exercises judgment and decision-making to provide support for the House Calls team of primary care professionals
